Transcript of Records
A transcript is a complete and unabridged copy of all academic work earned at Catholic Theological Union and includes only those courses attempted at Catholic Theological Union. The Course and grade information contained on the transcript is released only upon written consent from the student as required by the federal Family Educational Rights and Privacy Act of 1974 (as amended).
How to obtain a transcript
A student may obtain a transcript of his or her CTU record by applying in writing to the Office of the Registrar at least one week in advance of the date on which the transcript is needed. FAX requests are accepted, but transcripts will NOT be forwarded by FAX. You can download the form here or complete the online request form.
What information to include with a transcript request
Your request should include the full name under which you attended CTU, your date of birth, your date of last attendance, your social security number and your student ID number. Also include the complete name and address to which you want the transcript mailed, your current address, and a daytime telephone number. Your request must include your written (not electronic) signature to release your transcript.

On campus: Transcript request forms are available outside of the Office, Rm. 375, in the 5416 building.
Cost for processing a transcript
- Rush service (within one working day) is $12 per transcript.
- Regular service (within three working days) is $10 per transcript.
- Same Day service is $12 per transcript.
- Unofficial Transcript $9.
Remit Transcript fees by check or money order, made payable to Catholic Theological Union, attention to the Registrar’s Office. You can call the office at 773-371-5454 to pay by credit card.
Issue to student or send directly
Every transcript that is issued directly to a student is clearly so marked. Because most institutions will not accept a transcript that has been in the student’s possession, we urgently recommend the student request the Office of the Registrar to mail a transcript directly to the institution involved. Students who elect not to follow this recommendation are liable for any further charges for additional transcripts. Remember to sign your request.
